EMPLOYEE BENEFIT SUMMARYUpdated May 2009WORK WEEK Support, Inc. recognizes a 38 hour workweek, Monday – Thursday (or Tuesday through Friday for specific positions) for many positions and program areas, including residential services. Support, Inc.’s Day Program Services operates 5 days per week and many administrative positions also operate on a 5 day work week, Monday through Friday. Please contact Human Resources if you have questions concerning the workweek for specific positions. 401-K – THE STANDARD (Optional Profit Sharing Plan) Support, Inc. offers a 401-K for retirement savings through The Standard. Eligible employees may participate during the open enrollment period, 6 months from date of hire. Support, Inc. will match your own employee contribution, dollar for dollar up to 4% of your wage/salary. Increased employee contributions can be made. Employees are fully vested (own Support, Inc contribution) after 3 years of participation. DOMESTIC PARTNER COVERAGE Support, Inc. recognizes the designation of a domestic partner and the children of domestic partners for leave benefits and sick leave usage. Note: Health Insurance and Dental Health Reimbursement Accounts typically start 30-60 days from date of hire for full time employees (those individuals with a regularly scheduled 30+ hours per week). HEALTH INSURANCE – Kaiser Permanente Support, Inc. pays the majority of the employee health premium. Dependent coverage is available at the employee’s cost and can be paid for with a pre-tax, payroll deduction. DENTAL PLAN – Health Reimbursement Account Eligible employees may use to $1000.00/year for reimbursable dental services. Employees are responsible for paying their dentist directly and presenting documentation and receipt to Support, Inc. for reimbursement up to $1000.00/year. At no cost, there is an additional $500.00 total/year available for employee’s spouse and/or dependent children. LIFE INSURANCE – Anthem Life Coverage is fully paid for by Support, Inc. and the life insurance policy is in the amount of the employee’s yearly compensation. WORKER’S COMPENSATION INSURANCE - Pinnacol Assurance Support, Inc. provides a comprehensive worker’s compensation insurance program at no cost to employees to cover illness or injury sustained in the course of their employment. LONG TERM DISABILITY – Lincoln Financial Group Coverage is fully paid for by Support, Inc.. Long-term disability insurance provides partial income replacement for eligible employees who may become fully or partially disabled. SHORT TERM DISABILITY-The Paul Revere Life Insurance Company Optional coverage offered to eligible employees of which premiums are covered by the employee through approved payroll deductions. 125 CAFETERIA PLAN – FlexMagic Flexible Benefits Plan This plan allows an employee to set aside an amount from each paycheck, on a pre-tax basis, to be used for the cost of dependent child care and/or family health expenses not covered by insurance - i.e. co-pays, dental and vision expenses. The employee portions of premiums for health, dental and vision are paid for under this plan with pre-tax payroll deductions, an excellent way to save money with pretax contributions. SICK LEAVE Eligible employees earn 12 days of sick leave per calendar year. An eligible employee may use sick leave benefits for an absence due to his or her own illness or injury, or that of a child, parent, spouse or domestic partner of the employee. BEREAVEMENT LEAVE Eligible employees may use up to 3 days of paid bereavement leave per death for “immediate family” or for individuals whose relationship to the employee is similar to family. Please contact the HR office with specific questions. TIME OFF TO VOTE Support, Inc. provides up to 2 hours of paid time for employees to vote. CREDIT UNION – Bellco Credit Union and Columbine Credit Union Support, Inc. offers employees the option of participating in a credit union. Eligible employees can have a payroll deduction automatically deposited into a savings or checking account. Many different banking services are offered. DIRECT DEPOSIT Support, Inc. offers direct deposit of your paycheck into most major banks and credit unions. You can elect direct deposit at any time during your employment. VACATION BENEFITS Eligible employees earn paid vacation time which increases with the length of their employment, starting with 10 earned vacations days per year for the first 3 years of employment to a maximum of 25 earned vacation days per year after 10 years of employment. Vacation accruals are capped at 240 hours. CHILD CARE BENEFITS Support, Inc. provides child care assistance, when able through the use of the Flexible Spending Account, flextime scheduling, flexible leave and sick leave benefits. LEAVE Support, Inc. offers comprehensive plans for Medical Leave, Family Leave, Personal Leave, Military Leave and Pregnancy Related Absences. Please direct specific questions to the HR department. HOLIDAYS Support, Inc. will pay eligible employees for holidays, as approved annually by The Board of Directors. There are approximately 10 holidays recognized annually. PRE-PAID LEGAL PLAN/IDENTITY THEFT SHIELD Support, Inc. offers to it’s eligible employees an opportunity to purchase a pre-paid legal plan or identity theft plan at their own cost. HEALTH CLUB MEMBERSHIP – Bally’s Total Fitness Support, Inc. offers membership to Bally’s Total Fitness. Eligible employees are expected to cover ½ of the cost for monthly dues. CELL PHONE REIMBURSEMENT For those positions that do require the purchase and use of a cell phone, Support, Inc. provides a reimbursement for the purchase of such a calling plan. MILEAGE REIMBURSEMENT Support, Inc. offer’s mileage reimbursement to employees using their own vehicle in the performance of their job duties. GOING GREEN Support, Inc. offers monthly bus passes to employees who use public transportation to go to and from work. THE ABOVE IS INTENDED AS A SUMMARY OF BENEFITS OFFERED BY SUPPORT, INC.
