Our services are designed to support the person in those tasks important to living in the community, but which the person cannot perform independently. Our focus is on helping people get and keep adaptive skills and behaviors that are necessary to living in the community. Our expertise includes 24-hour supervised living, ADA issues, and Positive Behavior Supports. Adult Day ServicesAll Day Services encourage independent living, increase community integration and improve overall quality of life. All programs accentuate the individual's abilities rather than disabilities. People receiving Day Services include those with physical impairments, mental health problems, brain injuries, and developmental disabilities. Often, volunteer opportunities are incorporated into schedules so people can give as well as receive. A goal-directed program fosters independence, community participation, and self-sufficiency. Our programs provide a structured, individualized treatment program. Supervised LivingWe assist in developing a "community map" whereby home, work/day services, recreational facilities, and transportation are meaningfully related. Supervised Living and Supported Living services begin with the individual's hopes, dreams, and needs. Services are provided in the person's home, community, or neighborhood, or in a host home. Support, Inc. strives to provide community-based services that result in quality
lifestyles, characterized by integration into the community, a sense of belonging,
empowered choice-making, and opportunities for growth and learning. We measure our
performance in terms of consumer satisfaction and the achievement of quality outcomes. Through a systematic, regularly scheduled, comprehensive case review process, a
team of Support, Inc. professionals, referral case managers, and community support persons
evaluate the progress being made toward an individual's or family's goals. Specialty Services and ProgramsAssessment and Evaluation/Total Life Planning, Essential Lifestyle Planning.Community-focused service delivery is part of Support, Inc.'s vision of providing individualized services. It includes significant members of the community. Parents, family members, friends, associates from work, church, and local neighborhoods all play a part in the planning and delivery of comprehensive, coordinated services. Quality AssuranceOur efforts to be accountable and to have specific outcomes has resulted in the development of our Quality Assurance System. The results of the system, collected annually, assist us in improving our services. Nursing ServicesNursing Services are available to assist any individual who has a medical need. Nurses provide health counseling and education, and nursing assistance. They also obtain or clarify medical information and coordinate access to community medical services. The nursing staff, along with our Program Coordinators, coordinate evaluations and/or consultations by physical therapists, occupational therapists, adaptive technologists, and physicians. Psychiatric and Psychological ServicesOur psychiatric services are available to assist in the evaluation and regulation of current medication, as well as assistance with current symptoms. Psychological and counseling services are provided through our network of approved psychologists and counselors. Benefits AdministrationWe review existing benefits to ensure continued eligibility and utilization of all appropriate public assistance, including PASS plans, IRWE, and 1619(a) and (b) programs. We will help with reporting wages to Social Security and monitor eligibility for Medicaid and/or Medicare. |
