Employee Benefits

HEALTH INSURANCE:

KAISER PERMANENTE:  Majority of premium for the individual staff member is paid for by Support, Inc.

Total Cost:                   Varies per employee

Cost to Employee:        Approximately $85.00/month

(Note: Spouse and dependent children may be added to the policy, the cost for their coverage is the employee's responsibility.)

DENTAL REIMBURSEMENT: Each full time employee is eligible for up to $1000.00/year of dental services as reimbursed by Support, Inc. There is an additional $500.00/year for spouse and dependent children.

LIFE INSURANCE:

ANTHEM LIFE:  Premium cost covered by Support, Inc. No cost to the employee.  Pay out of life insurance in 1x annual compensation.

SHORT TERM AND LONG TERM DISABILITY INSURANCE:

Long Term Disability:  No cost to the employee.

Short Term Disability:  Employee can choose to purchase at their own cost.

VACATION BENEFITS: 

2 weeks of paid vacation per years in years 1-3 of employment

3 weeks of paid vacation per year in years 3-5

4 week of paid vacation per year in years 6-10

Unlimited vacation time for 10+ years of employment

SICK LEAVE BENEFITS:

Accrued at a rate of one paid day per month with a cap of

42 days.

401K RETIRMENT SAVINGS PLAN:

Employee becomes eligible after 6 month of full time employment.  After 6 months, all contributions to the plan are matched by Support, Inc. up to 4% of wage.

CAFETERIA PLAN:

Plan allow eligible employees to have deductions made from paycheck pre-tax to cover predicted medical and child care expenses. 

PREPAID LEGAL PLAN:

Employee may choose to purchase a pre-paid legal plan which provides legal services and/or identify theft protection. 

HEALTH CLUB MEMBERSHIP:

24 Hour Fitness or Bally’s Total Fitness.  Support, Inc. would pay for any initiation fees and ½ of the monthly dues.  The other half of the monthly dues are deducted from paycheck.

BUS PASS:

Reimbursement for monthly bus passes available to eligible employees with the appropriate receipt.

MILEAGE:

Mileage for work related transportation is reimbursed at .50 cents per mile.

HOLIDAYS:

Support, Inc. recognizes 10 paid holidays throughout the calendar year. 

CREDIT UNION:

Eligible employees are able to join Bellco Credit Union or Columbine Credit Union.
