February Family Caregiver of the Month!

We would like to congratulate and nominate Kalyn Berghoefer, for Family Caregiver of the month! Kalyn moved to Colorado last year to become the family caregiver to her younger brother, Jack. Being able to witness the bond between Kalyn and Jack is incredibly special. According to their Residential Coordinator, Kalyn is one of the sweetest and most kind-hearted people we’ve had the pleasure of getting to know. She advocates for Jack and makes sure his voice is the loudest in the room, always emphasizing that his choice matters. It’s been wonderful to see their relationship shine in their time with Support Inc.

One of the biggest events that Jack looks forward to is his triathlon, but he had to miss it this past year due to being sick. Kalyn and the family knew how important this was for Jack, so they created a triathlon specifically for him and he was able to participate while they cheered him on. Kalyn is always coming up with ways to celebrate Jack’s accomplishments and making sure he doesn’t miss out on the activities he loves. Kalyn, you have the sweetest heart and offer unwavering support to not only Jack, but everyone around you. Congrats on being nominated as our February Family Caregiver of the month!!!

  • We offer Supported Community Connections (SCC) and Specialized Habilitation (Spec Hab).
  • Our day programs provide structured Spec Hab and community activities each week.
  • SCC:
  • Community activities that build social skills, engagement, and independence.
  • Includes personalized outings and volunteer opportunities.
  • Spec Hab:
  • Site-based activities focused on self-care, communication, and daily living skills.
  • Supports personal growth through direct, structured assistance.

Where We Are Now:

  • Day programs operating in Pueblo, Aurora, and Loveland.
  • Offering outbound support in the Denver metro area and surrounding communities.
  • Expanding opportunities for individuals in residential support.

Where We’re Headed:

  • Immediately expanding to Greeley due to demand in Weld County.
  • Future locations include but not limited to, Colorado Springs, Jefferson County, Boulder County, Adams County.
  • Continuing to assess needs for future growth.

Ben Enjoys a Night to Shine!

One of our individuals in services, Ben, recently participated in the Night to Shine dance that is done through the Tim Tebow foundation.  Ben had a great time the week before the dance going to Men’s Warehouse with his Host Home Provider, Cindy Neumann, to get fitted for his tux.  The highlight in getting fitted was the “shinny shoes” (he had a difficult time when it came to returning those shiny shoes!).  It was hard for Ben to wait a week until he was able to ware his Tux and those special ‘shiny shoes’ but when it came time for the dance, it was worth the wait.  Ben had a great time!!  He loved getting all dressed up and getting a boutonniere pinned on him.  He enjoyed getting to eat Chick-Fil-A who provided food for the event.  He met up with others from his day program and was able to socialize with all of them.  Ben received a crown that he wears all the time! It is safe to say that this event is very important and special to a lot of people, and it was a great night for Ben!! Lily Moves to a Host Home!

We want to congratulate one of our individuals in services! This past month Lily moved from a Family Caregiver (FCG) setting to a Host Home setting.

Lily has had the goal of moving from her parents home for some time and now it is truly happening!! Lily and her FCG worked on money management, how to navigate her work schedule and both had made lists on what was important to each of them when finding a Host Home. Lily goes to a Day Program where she found a very interested Direct Support Professional who was hoping to become her Host Home Provider (HHP) full time. After a few meetings we quickly realized what a great fit this would be! The soon to be HHP asked her questions, but was also mindful to address any concerns and questions from Lily and her FCG. The HHP and Lily were very excited to see Lily’s goal of moving out of her family home was coming to fruition. This transition went so smoothly, Lily and her family had everything packed and ready to go before we arrived to help. They were clearly very organized. Lily wants to gain more independence and her FCG was very supportive of helping her achieve this goal. Lily’s new HHPs showed up to help her load her belongings in their truck. In a time that could have been stressful, this process was simple and encouraging to see Lily being put first in her goal of reaching more independence. The FCG followed us to the HHP’s home and helped unload Lily’s belongings, hugged her and said call me if you need anything. It was heartwarming to see how supportive both the FCG and HHP’s were in this transition. Congratulations to Lily and all for accomplishing this goal!

Clinical Team News & Updates

The new year is here and the Clinical Team has been working hard! In 2024 our clinical services increased and we were able to provide more care than ever before! We are looking forward to continuing to expand our clinical outreach this year. One area we would like to highlight is growing our group counseling services. With our new office location in Loveland, CO, we will be able to start offering group services and integrate them into our new day program there. We also continue to run multiple groups weekly at our Aurora Day Program location. Our group offerings range from emotional regulation, social skills, to healthy relationships and we will keep adding in new groups throughout 2025.

Happy Holidays From Your Quality Assurance Department!

December ushers in the holiday season including Christmas, Kwanzaa, Hanukkah and the New Year. This is often a time of reflection as we contemplate our successes and challenges over the last year. If we could sum up 2024 in a few words, it would be a ‘year of change’. Services for people with IDD went through large scale changes in this last year.

  • Case Management Redesign required Colorado case management agencies to provide case management to all 10 Home and Community Based Service (HCBS) waivers as a means to simplify access to long-term services and supports. Within case management redesign, the Community Center Boards (CCB’s) were also transitioning to conflict free casement in which they could no longer provide both case management and direct services. This year the last 2 cohorts of Case Management Agencies (CMA) transitioned.

 

  • Colorado Department of Public Health and Environment (CDPHE) abbreviated surveys: CDPHE spent this year conducting abbreviated surveys to catch up on several years of backlogged recertification visits to Program Approved Services Agencies (PASA’s). We had our recertification visit in April.

 

  • Updated/ New Regulations with the changes in Case Management Redesign came the need for the state to revise current regulations to align with the change to CMA’s. While the last minor revisions are going through final approval with the Medical Services Board, these updated regulations include changes for PASA’s.

 

As we look ahead to 2025, our system will have a few more large-scale changes.

  • NEW Person-Centered Support Plan (PCSP) & Colorado Single Assessment (CSA) tool to replace the current service plan (SP) and ULTC 1002. assessment. The new PCSP continues to outline the HCBS and long-term services and supports a person is receiving but the revised plan will be more person-centered and ensure better coordination of services. The new assessment will be a universal tool applicable to all people receiving HCBS services. Both tools will be implemented in 2025, HCPF is exploring a slow rollout and updates to the timing will be provided.

Kenny Ride on the Railroad!

Kenny recently took a fun weekend trip to the Georgetown loop railroad and had a blast! This has been a goal of his for about three years, so he was ecstatic that he was finally able to go. According to Kenny his love for trains started when he was a kid. He has always enjoyed watching movies that have trains because they look really cool. Kenny also usually likes to spend time watching YouTube videos on trains as well. HHP knew this was a goal of Kenny’s for a while and was able to take him up for the weekend. He even took some awesome pictures with Santa!
